Membership Plans
Individual
$299/month*
Personal doctor for less than $10 per day
*Annual commitment required.
*Payments can be made annually, semi-annually, or quarterly.
Family
$329/month
$100 Enrollment Fee
$279/month*
Each family member saves $240 per year
*Same rules as Individual plan.
*Applies to the following relatives: spouses, registered domestic partners, children, parents, grandchildren, grandparents.
Insurance
We accept all PPO insurance plans.
We do not accept HMO, Medicaid, MediCal, or CalOptima.
PPO coverage may vary depending on whether the physician is "in-network" or "out-of-network." We are "in-network" with the carriers identified below. If your plan is with a different carrier, we can still accept your insurance. However, you may want to consult your plan administrator to learn about your coverage for services rendered by an "out-of-network" physician.
